Otsuka est né le 15 mai 1962 à Palo Alto, en Californie. Elle a enseigné à l'Université de Columbia, à l'Université de Yale et à la Columbia University School of the Arts, obtenant un baccalauréat ès arts en 1984. Elle a ensuite obtenu une maîtrise en beaux-arts de l'Université de Columbia en 1999.
Son père travaillait comme ingénieur en aérospatiale et sa mère travaillait comme technicienne de laboratoire avant de donner naissance à Otsuka. Ses deux parents étaient d'origine japonaise, son père étant un issei et sa mère étant une nisei. Quand elle avait neuf ans, sa famille a déménagé à Palos Verdes, en Californie. Elle a deux frères, dont l'un, Michael Otsuka, enseigne à la London School of Economics.
Son premier roman When the Emperor was Divine traitait de l'internement des Américains d'origine japonaise pendant la Seconde Guerre mondiale. Il a été publié en 2002 par Alfred A. Knopf. Son deuxième roman, Le Bouddha dans le grenier (2011), parle de mariées japonaises.
Les romans de fiction historique d'Otsuka traitent des Américains d'origine japonaise. Ses livres attirent l'attention sur le sort des Américains d'origine japonaise pendant la Seconde Guerre mondiale.
Otsuka vit à New York.
